  • Proof of termination of prior marriages. This includes things like divorce papers, the death certificate of their former spouse, or a certificate of annulment (if applicable).
  • Proof of official name change, if any. Usually your marriage certificate will suffice, but if that’s not accepted, then a court order of name change will also work.
  • Military records. You must provide records of military services if you served in the military in ANY country (if applicable).
  • Photocopy of valid passport biographic data page.
  • Records of previous immigration problems. (If applicable.)
  • Police clearance certificates. You must acquire a police clearance certificate from all countries you’ve lived in.
