Proof of termination of prior marriages. This includes things like divorce papers, the death certificate of their former spouse, or a certificate of annulment (if applicable).
Proof of official name change, if any. Usually your marriage certificate will suffice, but if that’s not accepted, then a court order of name change will also work.
Military records. You must provide records of military services if you served in the military in ANY country (if applicable).
Photocopy of valid passport biographic data page.
Records of previous immigration problems. (If applicable.)
Police clearance certificates. You must acquire a police clearance certificate from all countries you’ve lived in.
